Are you able to demonstrate proficiency in the expression and comprehension of American Sign Language (ASL)? Do you have experience helping clients obtain services from social service agencies, health care organizations or governmental agencies?
The Community Based Long Term Care Area in the Human Services Department seeks a Senior Community Health Worker who is proficient in ASL. This position will assist hearing impaired co-workers and clients and requires both oral communication skills and sign language skills.
Working in a multi-disciplinary team located at various sites, this position will focus on serving persons with disabilities &/or seniors who need supports for remaining in their own homes or in the community rather than institutional settings. The team will share responsibilities to provide services with Medical Assistance (MA) Home Care &/or MA waiver environment.
Note: This position does require travelling to clients' homes using your own vehicle.
Hours are 8:00 to 4:30 but may require hours to be flexible to meet clients' needs.
The Primary duties of this position include:
-Share team responsibilities of provision of services in a complex Medical Assistance (MA) Home Care &/or MA waiver environment
-Provide assistance in the delivery of case management services
-Assist clients complete forms
-Assist clients by providing information on housing issues
-Assist clients transition to and from nursing homes or / and hospitals
-Locate community resources to resolve housing barriers
-Complete case documentation and maintain accurate record keeping in a computerized record database
-Provide follow-up and on-going support activities with clients, vendors, and service providers both by telephone and in -person.
The Ideal Candidate will have:
Completed two years of college in the Behavioral Sciences (such as Psychology, Sociology, Social Work)
Experience providing home based services to persons with disabilities &/or seniors
ASL proficiency
A valid driver's license with no more than two moving violations in the past three years
